Evaluations are required for Grant Programs and are to be completed by the end date of the Grant Agreement and returned to: The  Baraga County Community Foundation                                 100 Hemlock Street   Baraga,  MI 49908

The following information is required as a condition of any grant received:

Program Purpose (Provide a brief narrative of the purpose of your project, photos, examples of how the community foundation was mentioned in your public relations/printed materials, and/or press releases [supply sample copies], a financial report of grant monies spent, and any feedback that you feel may be valuable to the foundation.

Additional Information Required – Number of persons and age and geographical groups served, program type, and special interests served if applicable.
