The following information is required as a condition of any grant received:
The Baraga County Community Foundation must be mentioned as a supporter of your program/project in all of your program/project publications and promotions.
Program Purpose (Provide a brief narrative of the purpose of your project, photos, examples of how the community foundation was mentioned in your public relations/printed materials, and/or press releases [supply sample copies], a financial report of grant monies spent, and any feedback that you feel may be valuable to the foundation.
Additional Information Required – Number of persons and age and geographical groups served, program type, and special interests served if applicable.
